Personal Medicine Plus provides mobile prevention interventions for low-income
and/or rural members of health insurance and systems who are at-risk for
diabetes and hypertension. Unlike our competitors focused on disease
management, Personal Medicine Plus is a prevention management tool that
increases primary care utilization and provides access to prevention strategies in
between visits. Our mobile application provides patients with their own biometric information,
with interpreted and actionable messaging that allows for moments of
intervention. These moments are; the patient’s opportunity to take control of their
health or the prompt to visit a primary care physician before symptoms become
acute.
• The integrated social sharing through facebook specifically enhances adherence to our mobile health coaching tool and provides the opportunity for peer support and accountability. Facebook sharing enables the positive health behaviors of our users to be shared virally with their friends and network, enabling progressive health improvements and positive behavior change within communities.
